We ask that you pay for your portion of your services at the time of your visit. For your convenience, we accept cash, personal checks, Visa, MasterCard, American Express and Care Credit.

Medicaid patients will need to bring a copy of their current Medicaid card for each visit. If a co-payment is required, it must be paid at the time of service.
All our physicians accept assignment for Medicare patients.
If you are being treated for a Worker's Compensation injury, please bring your case number and the name and address of the employer's Worker's Compensation Carrier at the time of service. Worker's compensation now requires 2 weeks authorization for medical tests and surgeries. These approvals must be in writing.
